Graduate School Registration and Forms
Health Center Course Offerings
Fall 2008 course registration will begin at noon on Monday,
July 7, 2008, for degree students and on Monday, July 14, 2008, for non-degree students. Students enrolled in a
degree program will be allowed to register on-line. Non-degree students must use the paper registration form located at the end of the on-line catalogue. Please note that the final date
for registration and payment without penalty is Monday, September
8, 2008. Students are strongly encouraged to register prior to the start of classes.
Immunization Requirements
Department of Occupational and Environmental Medicine Immunization Requirements
All incoming students must satisfy the immunization requirements defined below. Submitting the forms does not guarantee compliance and eligibility to register for
courses. Students are encouraged to contact Occupational Medicine after the forms have been submitted to ensure compliance with regulations and clearance for registration.
Connecticut State Law requires that all full-time and/or part-time matriculating students born after December 31, 1956 who are enrolled in post-secondary schools be
protected against measles and rubella. Minimum requirements are: one dose of rubella vaccine given on or after the student's first birthday and one dose of measles vaccine given after
January 1, 1969 and on or after the student’s first birthday.
Students born after December 31, 1956 will be required to show proof of two doses of measles-containing vaccine. One dose must have been administered on or after January 1, 1969 and
the second dose must have been administered on or after January 1, 1980. The student has the option of obtaining a lab test of immunity to both diseases: if both are positive, the
physician is to indicate the date and result of each blood test on the chart on the immunization form (PDF).
Tuberculosis - must have 2 Mantoux tests 2 weeks apart within the last 12 months or show proof of annual Mantoux testing. If ever positive, test proof of chest x-ray.
Forms
The Registrar's Office at the University of Connecticut Health Center acts as a liaison to the Graduate Records Office on the Storrs campus. All official paperwork should be filed
through the Registrar's Office on the Health Center campus, Room LM035, MC1827, regardless of the address on the forms.

Transcript Requests
To request an official transcript of Graduate School work, students must make a written request to the Registrar's Office on the
Storrs campus. Requests may be faxed to 860-486-0062 and must include name, current address, date, social security number, phone number and signature along with where the transcript is
to be sent. There is no charge for transcripts. Unofficial transcripts of Graduate School work are available from the Registrar's Office on the Health Center campus.
